Suay Memory Quilt Form

The following is a memory quilt form for those living outside the Los Angeles area. If you reside in the L.A. area, please visit our Frogtown Shop at 2915 Knox Ave. LA, CA 90039 to drop off your textiles. Please fill out each section below accordingly and accurately. Once submitted, Suay will send you an invoice within five business days to review and process to move forward with mail-in memory quilt service. As always, we thank you for extending the life of your garments with Suay.
